Baseball Camp Evaluation Form

If you have been to a baseball fantasy camp, I want your opinion.  Most camps are a great experience, some leave something to be desired.  This is an opportunity for you to tell the world what a great time you had or what you didn't like about the fantasy camps you attended.  Information for each camp will be compiled and averaged to determine an overall camp rating.  Most people I've talked to all think their camps were outstanding.  In an effort to help others decide on the best camp for them, I've developed this form as a guideline to evaluate the camps.  PLEASE REVIEW Evaluation Form Information and Explanation before filling out the form.  No camp qualifies as excellent in every category.  AOL users, you may have to fill out the form, print it and then fax it.  AOL does not process even the most generic forms very well.

If the camp did not provide any of the items listed below, leave it blank. If you leave a space blank it will count as zero points for that item so please respond to each question.  If you wish, you may print this form and fax to me (707-937-2349).
